What is Home Health Care? Home health care is a service to the recovering, disabled, or chronically ill person providing treatment and/or effective functioning in the home environment. Home care is appropriate whenever assistance cannot be easily or effectively provided by a family member or friend on an ongoing basis for a short or long period of time There are many situations and conditions for which home care is appropriate. Services such as catheter implementations/maintenance, wound dressing, IV injections, etc. are among the services that can be performed in the home. Recently, "hi-tech" services such as chemotherapy, TPN, and other IV therapy techniques have been recommended for home use. Why Consolidated Health Care? We provide quality nurses, home health aides, physical, occupational, and speech therapists. We also offer nutritional guidance and medical social services to help return our clients to their maximum level of health. We offer infusion therapy in our lineup of quality home health services. Our trained staff will work with our client's physician to develop the best plan for maintaining the continuum of care from the client's hospital stay to their eventual return to health in the home setting. Our objective is not only to provide cost effective service, but also to provide the service in the most comfortable place, the client's own home. Home Health Aide Home Health Aides are trained and supervised by the Registered Nurse to provide personal and health care services such as:
Physical Therapy Physical Therapists will teach and motivate the client to perform the exercises required for their recovery. The use of necessary exercise equipment will also be explained. In addition, the therapist may elect to use physical agents such as heat, massage, hot packs, and impulse therapy to help achieve maximum range of motion. Occupational Therapy This form of therapy is an important segment in the client's total care. Occupational therapy helps improve physical and emotional well-being by acclimating the client to all the necessary basic living skills they may have lost due to illness. The therapist is in consultation with the physician and will evaluate, set goals, and initiate treatment. By involving the client's family and other concerned individuals in the program, we will accomplish a smooth transition from the hospital environment to the home and the client's family. Speech Therapy An experienced and qualified therapist will come to evaluate, plan, and initiate a treatment program tailored to the client's needs in speech, hearing, and language problems. Teaching can be accomplished in consultation with the clients family, physician, and other allied health care team members. Nutritional Teaching Diet is an important factor in the overall recovery. All Certified Nursing Services will ensure that strict ADA guidelines are followed when so ordered by the attending physician. After the need for a special diet has passed, nutritional teaching will focus on food that is easily prepared and reflects a healthier lifestyle. Medical Social Services A trained Medical Social Worker will assess the effect of social and emotional factors in relation to the clients illness, treatment, and convalescence. The teaching will improve the clients ability to accept and adjust to the illness. Mission and Philosophy Consolidated Health Care Services dedicates its services to preventing illness and promoting health, and providing quality care which will enable each client to return to the maximum attainable level of health as quickly as possible or provide comfort and preparation for his/her impending demise. We believe that health care at a skilled level can be provided to the aging population within their own homes, thereby assisting them in maintaining their self esteem and family relationships, and avoiding or delaying institutionalization. We believe that the cost for this care should be controlled so that it is reimbursable by third party payers or paid by the client without undue financial hardship. We believe that Consolidated Health Care Services should, when the need exists, provide voluntary services to the community for the prevention of illness and health education. We recognize that health care is the right of every individual regardless of age, sex, race, color, creed, national origin, or economic status of life. |
